Creating Account

I'm trying to create an account for my mom, however when she puts in her previous carrier information, it is saying there is an error with the address. First it was their street, then it was the state & now her phone is locked & not in use. Has anyone ran into this & how did you fix it? Using the chatbot did not help much.
