It has been just over 3 years so memory isn't the greatest but I know the card did not come installed in the phone since you need to install it and activate it though the app. Now what I can't remember is if the card came in the same package with the phone or in a separate envelope but then they arrived the same day.
I will point out that this was when Visible sent both cards and phones next business day or 2 day shipping. Now phones are still shipped the same but the cards are different shipping methods depending on the plan. The base plan the cards are shipped regular USPS and can take up to a week. The plus plan cards are still shipped like phones. If you order a phone and a card for the base plan I am not sure if they will ship the same faster method or if the card will ship slow style and the phone ship fast.
