Can't activate phone - New service Declined

I have successfully activated and ported over 4 phones using eSim. I have 1 more and every time after entering the payment  and submitting it says declined and them service cancelled. This is an iPhone 11 from Verizon (as were all the rest of them I brought over). I called Verizon just to confirm the phones were unlocked and not frozen. All is good to transfer.  I have chatted with Visible many times and no resolution. I have uninstalled the app, rebooted phone etc.. Any thoughts?
